Privacy Policy

  • We are available to serve you, answer questions and schedule appointments during the following business hours:
    • Monday – Friday: 8 a.m. – 4:30 p.m.
    • Closed for lunch from 12:15 p.m. – 1:15 p.m.
  • We have set aside the following hours for blood pressure checks, INR’s and necessary injections.(Please note these are work-ins and may be subject to a wait time):
    • 8:30-11:00 a.m. and 1:30-4:00 p.m., Monday through Friday. Please make an effort to come when your prescribing doctor is in the office to maintain continuity of care.
  • Please call your pharmacy for prescription refills and allow THREE BUSINESS DAYS for refills to be completed. Refills on controlled medications will be done during office hours only. You may be required to sign a “controlled substance agreement”.
  • Prescriptions that you are taking on orders from another physician need to be authorized through their office.
  • Allow 7-14 business days for any paperwork that needs to be completed by your physician.  All forms are subject to additional fees $10-$20.
  • Please do not call requesting test results that your physician has ordered, unless you have not heard from us 1 week after testing. You will be given information on obtaining your results at your office visit.
  • New patients are required to arrive 30 minutes prior to appointment time to complete necessary paperwork. Late arrivals may necessitate your appointment being rescheduled.
  • Established patients are to arrive at least 15 minutes prior to appointment time. Late arrivals may necessitate your appointment being rescheduled.
  • Bring all of your medications, including over-the-counter, vitamins, and herbs, to each of your appointments.
  • Please DO NOT WEAR cologne, perfumes, lotions or any scented items, as many of our patients and staff have allergies and/or medical conditions that cannot tolerate these types of fragrances and the chemotherapy unit is next door to the office.
  • Please turn off cell phones while in our office.
  • If your insurance requires referrals, please contact our office at least one week prior to the date of your appointment with the following information:
    • Name of physician you need a referral to see
    • Physician’s phone and/or fax number
    • Diagnosis
    • Appointment date and time
